Online appointment requests are available for pregnancy tests, limited ultrasounds, and abortion consultations. For any other service or additional appointment times, please call us at 336-736-8878 or text 336-948-1173.

Please note, this is a request for an appointment, not a confirmation of appointment. Your appointment is not confirmed until you hear from a staff member. Be sure to include a phone number so we can contact you.

Fill in the blanks of the appointment request form with your contact information and check the box stating we may contact you directly to confirm the appointment. In the Comments Box of the appointment form, please include the following:

The first day of your last menstrual period
If you have had a pregnancy test, and if you have, the result
Type of appointment needed
- Abortion Consultation
- Pregnancy Confirmation
- Ultrasound
- For STI services please call the center.
